Participation & Incentive: Participation in this survey is entirely voluntary. As a token of our appreciation, all participants who received an invitation to participate and who completed the survey will receive a $100 Amazon gift card. This incentive is by no means compensation for your information but a gesture of gratitude for your time and insights.

Complete the Entire Survey: To qualify for the $100 Amazon gift card, you must complete all sections of the questionnaire. Partial or incomplete responses will not be eligible.

By Invitation: Only those invited to participate will qualify for the incentive. If an individual did not receive an email as an invitation to participate yet completed the survey, that survey will be disallowed, and the individual will not receive the incentive.

Email Address: We will send the $100 Amazon gift card to the email address we used to contact you. As an alternative, you can opt to provide a different email address. If you provide a different email address, then this is where we will send the gift card should you qualify.

Privacy is Our Priority: We value your privacy. All the information you provide in this survey will remain confidential and will only be used internally.
